      目的 基于数据挖掘方法探讨骨关节炎(OA)患者同型半胱氨酸(Hcy)的变化与免疫指标、炎性指标、代谢指标之间的相关性、危险因素和关联规则。方法 整理2012年6月至2018年12月OA合并Hcy异常的住院患者的病历资料,研究其代谢指标:高密度脂蛋白胆固醇(HDL-C),低密度脂蛋白胆固醇(LDL-C),总胆固醇(TC),三酰甘油(TG),α1-酸性糖蛋白(α1-AGP),丙氨酸氨基转移酶(ALT),天门冬氨酸氨基转移酶(AST),肌酐(Cr),尿素氮(BUN);免疫指标:免疫球蛋白A(lgA),免疫球蛋白G(lgG),免疫球蛋白M(lgM),补体C3,补体C4;炎性指标:红细胞沉降率(ESR),超敏C反应蛋白(hs-CRP)。运用SPSS 22.0、SPSS Clementine 11.1 Aprior等统计学软件对指标进行相关性分析、关联规则分析、二元 logistics 回归分析。结果 共纳入符合研究要求的患者1 430例。①Hcy升高有281例(19.65%)。②相关性分析表明,Hcy与TC呈负相关(P<0.05);Hcy与α1-AGP、hs-CRP、Cr以及BUN呈正相关(P<0.05或P<0.01)。③二元logistics回归分析表明,α1-AGP、Cr、BUN均为Hcy的危险因素。④关联规则分析表明,设置关联规则支持度>10%,置信度>30%,提升度>1。经Aprior模块分析,Hcy异常与Cr和ESR的升高有关、与HDL-C的降低有关。结论 19.65%的OA患者Hcy异常,α1-AGP、Cr和BUN作为Hcy异常变化的危险因素。

      Objective To explore the correlation between the changes of homocysteine(Hcy) and immune ,inflammation and metabolic in patients with osteoarthritis (OA) based on data mining methods.Methods To organize the medical records of hospitalized patients with OA and Hcy abnormalities from June 2012 to December 2018,and to study their metabolic indicators:high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),total cholesterol.(TC),triglyceride (TG),α1-acid glycoprotein (α1-AGP),alanine aminotransferase (ALT),aspartate aminotransferase (AST),Creatinine (Cr),urea nitrogen (BUN).immunological indicators:immunoglobulin A (lgA),immunoglobulin G (lgG),immunoglobulin M(lgM),complement C3,complement C4; inflammation index:erythrocyte sedimentation rate (ESR),hypersensitive C-reactive protein (hs-CRP).Using SPSS 22.0,SPSS Clementine 11.1 Aprior and other statistical software to carry out correlation analysis,association rule analysis and binary logistics regression analysis.Results A total of 1430 patients who met the study requirements were included.Serum Hcy was increased in 281(19.65%).Correlation analysis showed that Hcy was negatively correlated with TC; Hcy was positively correlated with α1-AGP,hs-CRP,Cr and BUN.Binary logistic regression analysis showed that α1-AGP,Cr and BUN were risk factors for Hcy.Analysis of association rules showed that the association rule was set to>10%,the confidence was >30%,and the promotion was >1.According to the Aprior module analysis,the increase in Hcy value was related to the rise of Cr and ESR and to the decrease of HDL-C.Conclusion 19.65% of patients with OA have abnormal Hcy.α1-AGP,Cr and BUN are involved in the abnormal change of Hcy.
