      目的 探讨血脂与1型糖尿病肾脏疾病(DKD)的关系。方法 采用横断面回顾性研究177例1型糖尿病(T1DM)患者相关资料,按照尿白蛋白/尿肌酐比值(UACR)水平分为3组:正常白蛋白尿组、微量白蛋白尿组和大量白蛋白尿组。结果 (1)与正常白蛋白尿组比较:大量白蛋白尿组三酰甘油、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)均明显升高,P<0.05;(2)TC与舒张压(DBP)、UACR、体质指数(BMI)呈正相关,与C肽水平呈负相关;高密度脂蛋白胆固醇与糖化血红蛋白(HbA1c)呈负相关,与C肽呈正相关;LDL-C与DBP、UACR、HbA1c、BMI呈正相关;极低密度脂蛋白胆固醇(VLDL-C)与年龄和UACR呈正相关。(3)logistic回归分析结果显示TC、LDL-C是影响DKD发生的独立危险因素。结论 脂代谢异常参与T1DM患者DKD的发生和发展。
英文摘要:
      Objective To analyze the relationship between lipid metabolism and urinary albumin/urinary creatinine ratio (UACR),and to evaluate the value of plasma lipid level in predicting the occurrence and development of diabetic kidney disease(DKD) among type 1 diabetic patients(T1DM).Methods Based on retrospectively cross sectional study,177 patients with T1DM were enrolled in this study.Patients were divided into three groups according to UACR:normo-albuminuria group (n=115),microalbuminuria group(n=46),macroalbuminuria group (n=16).Results (1) Levels of cholesterol(TC),triglyceride and low density lipoprotein cholesterol(LDL-C) in macroalbuminuria group were higher than those in normo-albuminuria group (P<0.05).(2)The TC level had the positive correlation with diastolic pressure(DBP),body mass index(BMI)and UACR,and had the negative correlation with serum C-peptide.High density lipoprotein cholesterol had the positive correlation with serum C-peptide,and had the negative correlation with glycosylated hemoglobin(HbA1c).LDL-C had the positive correlation with DBP,UACR,HbA1c and BMI.Very low density lipoprotein cholesterol had the positive correlation with age and UACR.Logistic regression analysis showed that TC and LDL-C were independent risk factors for DKD.Conclusions Impaired lipid metabolism is the independent risk factor of the occurrence and development of DKD in patients with T1DM.
