      目的 探讨血浆总同型半胱氨酸水平对健康人群脑血流动力指标的影响。 方法 选取230例体检人群,记录研究对象的年龄、性别、身高、体质量、腰围、收缩压、舒张压、血脂、脑血流动力学指标,计算体质指数(BMI)。根据血浆同型半胱氨酸(Hcy)水平将230例研究对象分为健康对照组(Hcy<15 μmol/L)和高Hcy组(Hcy≥15 μmol/L),比较两组各项指标。结果 高Hcy组患者在体质指数(t=-3.470,P=0.001)、收缩压(t=-3.583,P<0.001)、舒张压(t=-2.898,P=0.004)、低密度脂蛋白(t=-2.756,P=0.006)等方面明显高于健康对照组,差异有统计学意义。高Hcy组两侧脑血管最大流速(Vmax)均值(t=3.043,P=0.003)、最小流速(Vmin)均值(t=4.358,P<0.001)、平均流速(Vmean)(t=4.191,P<0.001)、可扩张度(DI)均值(t=2.829,P=0.005)均低于健康对照组,差异有统计学意义。而外周阻力(RV)均值(t=-5.247,P<0.001)、特性阻抗(Zcv)均值(t=-3.311,P=0.001)、脉搏波速(Wv)均值(t=-3.369,P=0.001)、临界压力(CP)均值(t=-2.237,P=0.026)均高于健康对照组,差异有统计学意义。高Hcy组的脑血流动力积分平均值明显低于健康对照组平均值(χ2=5.384,P=0.020),差异有统计学意义。随着血浆同型半胱氨酸水平升高,脑血流动力学积分值呈显著降低趋势,两者呈负相关(r=-0.335,P<0.01)。结论 血浆同型半胱氨酸水平升高可导致脑血流动力学指标及其积分值的变化。

      Objective To investigate the effect of plasma homocysteine level on cerebral hemodynamics indexes in healthy subjects.Methods A total of 230 healthy people were enrolled in this study.Age,sex,height,weight,waist circumference,systolic blood pressure,diastolic blood pressure,blood lipid and cerebral hemodynamics The body mass index (BMI) was calculated.According to plasma homocysteine level,230 subjects were divided into normal group (Hcy<15 μmol/L) and high Hcy group (Hcy≥15 μmol/L).Compare two groups of the indicators.Results The body mass index (t=-3.470,P=0.001),systolic blood pressure (t =-3.583,P<0.001),diastolic blood pressure (t=-2.898,P=0.004),and low-density lipids Proteins (t=-2.756,P =0.006) of patients with high Hcy were significantly higher than those of the healthy control group,and the differences were statistically significant.The mean flow velocity (Vmax) of the cerebral vessels on both sides of the high Hcy group (t=3.043,P=0.003),mean of the minimum flow velocity (Vmin) (t=4.358,P<0.001),mean flow velocity (Vmean) (t=4.191,P<0.001),the mean dilatability (DI) (t=2.829,P =0.005) was lower than that of the healthy control group.The difference was statistically significant.Peripheral resistance (RV) mean (t=-5.247,P<0.001),characteristic impedance (Zcv) mean (t=-3.311,P=0.001),pulse wave velocity (Wv) mean (t=-3.369,P=0.001).The mean of critical pressure (CP) (t=-2.237,P =0.026) was higher than that of the healthy control group.The difference was statistically significant.The mean value of cerebral hemodynamics in the high Hcy group was significantly lower than that in the healthy control group(χ2=5.384,P=0.020),and the difference was statistically significant.With the increase of plasma homocysteine level,the cerebral hemodynamic integral value showed a significant decrease trend,and there was a negative correlation between them (r=-0.335,P<0.01). Conclusion Elevated plasma homocysteine level can lead to changes of cerebral hemodynamics indexes and its integral value.
