      目的 探讨人参果总皂苷对慢性心力衰竭合并室性心律失常患者内皮细胞功能的影响。方法将接受治疗的82例慢性心力衰竭合并室性心律失常患者采用随机信封法分为观察组和对照组,各41例。对照组接受常规治疗,观察组在常规治疗的基础上联用人参果总皂苷。治疗12周后观察疗效、窦性心律维持情况、心功能指标、血管内皮功能指标及血管活性物质指标。结果 观察组临床疗效、窦性心律维持情况略好于对照组,但差异无统计学意义(P>0.05)。治疗前,两组心输出量(SV)、左室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、肱动脉内皮依赖性舒张功能(FMD)、肱动脉非内皮依赖性舒张功能(NMD)、内皮素-1(ET-1)、血栓素-2(TXB2)、一氧化氮(NO)水平对比,差异无统计学意义(P>0.05)。治疗后,观察组SV、LVEF、FMD、NMD、NO明显高于对照组,LVESD、LVEDD、ET-1、TXB2明显低于对照组,差异有统计学意义(P<0.05)。结论 人参果总皂苷辅助治疗慢性心力衰竭伴室性心律失常,有助于提升患者预后,这可能与其对血管内皮细胞功能的保护作用有关。
英文摘要:
      Objective To investigate the effects of total saponins from Panax ginseng on endothelial function in patients with chronic heart failure and ventricular arrhythmia.Methods Eighty-two patients with chronic heart failure and ventricular arrhythmia were randomly divided into the observation group and the control group,with 41 cases in each group.The control group received routine treatment,while the observation group was treated with total saponins of ginseng fruit on the basis of routine treatment.After 12 weeks treatment,the curative effect,maintenance of sinus rhythm,cardiac function indexes,vascular endothelial function indexes and vasoactive substances were observed and compared between 2 groups.Results The clinical efficacy and maintenances status of inus rhythm in the observation group were slightly better than those in the control group,but the differences were not statistically significant (P>0.05).Before treatment,stroke volume (SV),left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter (LVESD),left ventricular end-diastolic diameter (LVEDD),flow-mediated vasodilation of brachial artery (FMD),non-flow-mediated vasodilation of brachial artery (NMD),endothelin-1 (ET-1),thromboxane-2 (TXB2),and nitric oxide (NO) between 2 groups were similar,there were no significant differences (P>0.05).After treatment,SV,LVEF,FMD,NMD and NO in the observation group were significantly higher than those in the control group,and LVESD,LVEDD,ET-1 and TXB2 in the observation group were significantly lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion Total saponins of Panax ginseng can help to improve the prognosis of patients with chronic heart failure and ventricular arrhythmia,which may be related to its protection on vascular endothelial cell function.
